What is confirmed
Every store listing for Clive Barker's Hellraiser: Revival uses the same sentence: Aidan must wield the powers of the box to survive his bargain with the infamous Pinhead. The PlayStation Blog developer diary identifies Pinhead by his other name, the Hell Priest, when quoting the original film: "Explorers in the further regions of experience. Demons to some, angels to others."
That quote is the whole franchise thesis, and Saber's game director builds the game's morality around it. The diary argues that the Cenobites are not easily read as morally bad, describing them as monks who explore sensation at the extreme edges of pain and pleasure.
So Pinhead is not presented as the villain. In this story he is the party Aidan has to survive a bargain with, which is a very different dramatic role from a slasher antagonist.
What is not confirmed
⚠️ Unconfirmed. No first-party page we can read names the performer playing or voicing the Hell Priest in this game. A community thread mentions a behind-the-scenes video featuring Doug Bradley, who played the character across most of the film series, but that thread is a single community source and does not confirm casting. This wiki will not state casting until Saber or a platform holder does.
The same caution applies to the general question of a Pinhead-centric mode or boss fight. Nothing official describes the structure of his appearances.
Where he fits in the story
The order of events, per the dev diary: Sunny activates the Configuration, the Cenobites appear and tear her in half, and Aidan learns that her soul can be saved and that the Cenobites are integral to that. Pinhead is the figure on the other side of that arrangement. The rest of the premise is on the Story page and the cast list on Characters.
FAQ
Is Pinhead the main villain?
Not in the way slasher villains usually are. The dev diary explicitly frames the Cenobites as explorers rather than as morally bad, and the human cult, the Scarlet Church, is the more conventional antagonist.
Can you fight Pinhead?
Nothing official describes a fight, a chase or any other encounter structure. The confirmed phrasing is that Aidan must survive his bargain with him.
Is this connected to the films' continuity?
The game is described as a new chapter in the Hellraiser canon rather than a remake. Franchise history is on Series Background.
