What the ratings say

The North American rating is Mature 17+, with these six descriptors, listed identically on the Xbox and PlayStation pages:

That is an unusually full set, and it is the honest summary of what the game contains. The Nintendo listing states "For ages 17 and up" on its own page.

This is a Hellraiser game, and Hellraiser has always been about the overlap of pain and pleasure. Nothing in the rating is surprising for the licence; what is notable is that Saber did not soften any of it for a game release.

The nudity question

⚠️ Unconfirmed. Community playthroughs of the demo report nudity present in cut scenes and during gameplay, and report that no nudity option was found in the settings menus they opened. Multiple forum threads ask for a toggle, and there is no first-party answer. Whether the launch build has one is unknown.

Two related community threads also discuss perceived censorship and regional differences, including one asking about a German release. Nothing official has been said about regional cuts, so this wiki does not claim any.

Trigger warnings

Playthroughs report that the demo opens with content advisories, including a photosensitive warning, before play begins. That is consistent with an M-rated title and is worth knowing if you are sensitive to flashing imagery or are planning to stream the game.

Streamers should note that platforms set their own rules for nudity and extreme gore, which is separate from what the rating permits. The descriptors above are the best predictor you have of what the camera will show.

Who should skip it

If any of the six descriptors is a hard limit for you, this is not the game to make an exception for. The rating is not a marketing device here; it is a description. The franchise premise is built on exactly the content those words describe.
